EDIT: Detailed testing without mods proven, that currently the incoming multiplier is 4. Which means, that on survival you take 4 times damage of normal, and very hard works properly with the double damage on player. Sadly, lack of fast travel is one of the major points of FO4's Survival mode. Local leader does help for lines of supply - but charisma 6 is a significant investment on a build. In all honesty, I generally don't finish survival playthroughs - I get to the point where it's comfortable and then quit because of fast travel.Then survival mode is not for you.
