The clinical, haematological, and biological abnormalities associated with this rare disorder indicate that it affects the membrane integrity of red blood cells. The most commonly ordered routine blood tests would not detect it, but Rh null people are known to have anemia and abnormal red cell shapes, so it's possible a routine CBC would show anemia and might lead to some further investigation that would eventually lead to the answer.
